Job overview

Following a successful application for an SBRI Grant, funding has been provided to assess the impact of the my COPD app and a blended approach to pulmonary rehabilitation (PR) within Cornwall Partnership Foundation Trust, providing a ‘digital-PR' option to isolated communities and increasing the capacity and access to PR. PR is an exercise and education programme designed for people with lung disease who experience symptoms of breathlessness. PR focuses on tailored physical exercise and information that helps people to better understand and manage their condition/s and symptoms.

myCOPD is an app which helps people with COPD to manage their condition effectively and thereby reduce the number of healthcare visits they need. It allows for key aspects of disease management to be provided remotely, based on a person’s self-assessment. MyHealth have collaborated with the West of England AHSN to manage this project and real-world observation study.

We are looking for a Respiratory Care Co-ordinator to coach patients how to use the myCOPD app as well as providing a telephone service to proactively encourage the use of the PR section of the app. The post is fully funded for one year, starting in May 2023. This role can be completed by health care assistants or rehab assistants.

Secondment will be considered if agreed with current line manager prior to application.

Main duties of the job

  • Attend necessary training for myCOPD and be familiar with how the app works.
  • To attend pulmonary rehabilitation assessment and reassessment days with the community respiratory team, consent patients to the research study, coach patients how to use the app and safely use digital technology and the internet.
  • Pro-actively work with patients on assessment days to on board them on to the app in person.
  • Coach patients on how to use myCOPD, working with them to understand and overcome individual limitations and difficulties in using the app.
  • Through remote consultations, to work with patients that are not actively using the app and use Motivational Interviewing techniques to understand and overcome patient barriers to completing the PR home exercise plan.
  • Work collaboratively with the Community Respiratory Team, Digital Health Adviser and Cornwall digital inclusion officer to access digital devices if patients do not have their own.
  • Report to the service team lead and highlight any issues that arise.
  • Communicate with key clinicians involved in the patient care to highlight any specific concerns that become apparent during the coaching sessions.
  • Work collaboratively with the team lead to develop local processes for implementing and embedding the app.
  • Be responsible for data collection to support evaluation, inputting into an electronic data capture which will be provided.
  • Complete GCP training and consent patients on to the study, working with Research and Development colleagues
